What is Palliative Care?

Palliative care is specialized medical support for people living with serious illness — focused on
relief from symptoms, clarity around choices, and care aligned with what matters most. It's not
hospice. It’s care and support while you’re seeking treatment. It's not about giving up.

Who benefits most from palliative care?

If you, or your loved one, have a serious illness and need extra
support, you might benefit from Palliative care.

Who Qualifies for Palliative Care?

Individuals with serious, life limiting illness and:

  • Poor symptom control
  • Recent hospitalization or long-term care admission
  • Goals of care conflicts 
  • Declining functional status
  • Extensive care coordination or social needs

What patients qualify?

  • Patients with Advance Cancer (Stage III or IV)
  • Patients with Renal Disease (Late stage CKD or ESRD)
  • Patients with Congestive Heart Failure (CHF)
  • Patients with Progressive Degenerative Neurological Disorders (MS, MD, Parkinson’s, ALS, Late Stage Dementia)
  • Patients with COPD
  • Patients with Advanced Liver Disease 
(Elevated INR)
  • Patients with Aids/HIV (CD4 count <200)
