Referral Process Step 1:  Does This Person Qualify?

Refer a patient for assessment if they are living with a serious illness and may need more support at home.

Good candidates may have a serious illness, such as:

  • Cancer
  • Congestive heart failure
  • COPD
  • Kidney disease
  • Liver disease
  • Parkinson’s or another serious neurological condition
  • Alzheimer’s or dementia
  • Diabetes
  • Stroke
  • AIDS
  • Or other serious pediatric conditions

Plus signs that quality of life or function is declining, such as:

  • Difficulty with daily activities like bathing, dressing, walking, or moving around
  • Frequent emergency department use, including 2 or more ER visits in the last 6 months
  • A recent hospital stay, including 1 or more hospitalizations in the last 12 months
  • Worsening symptoms, caregiver strain, or increasing need for support at home

Referral Process Step 2:  How to Refer

Not sure if the patient qualifies? Refer for an assessment.

A perfect match is not required. Clinical judgment may be used when a patient’s condition suggests they could benefit from community-based palliative care.

Referral Process Step 3: What happens after you refer?

  • Vynca’s outreach team connects with the patient to determine their interest in the program and completes a screening questionnaire.
  • If patient is eligible for the program, an in-person assessment is scheduled at the patient’s home.
  • A Vynca clinician meets with the patient in person and completes a full assessment of symptoms, needs, goals, and care preferences (no prior approval needed for initial assessment).
  • Assessment results are sent to the patient’s health plan within 5 business days for authorization.
  • If approved, the patient receives team-based palliative care for up to 6 months, with reassessment every 6 months or sooner if their condition changes.
  • Vynca’s clinical team will reach out to you, as needed. Please feel free to reach out to the team to collaborate.

Vynca works alongside the patient’s existing doctors and care team.

Community-based palliative care does not replace the patient’s providers, specialists, or current treatments. Vynca extends the reach of community providers and specialists into the home with additional support for patients and families through whole person, interdisciplinary team care.

01. When should a patient be referred to palliative care?

Patients can benefit from palliative care at any stage of a serious illness and do not need to stop curative treatment.

Does a patient need a terminal diagnosis?

No. Palliative care is available to patients living with serious illness and can be provided alongside ongoing treatment.

Is palliative care covered by New Jersey Medicaid?

Community-Based Palliative Care is available to eligible NJ FamilyCare members as a Medicaid benefit beginning in 2026.

Can patients receive palliative care and continue treatment?

Yes. Home-based palliative care works alongside existing specialists and primary care providers.

Will my primary care provider change?

No. You can keep your current providers. We help you with clinical, behavioral, and social issues between your doctor visits.

How do I sign up?

Please fill out a simple form with some details about you. We will check your health plan coverage and clinical eligibility and then help schedule your first visit.

What happens after I sign up?


We’ll check if you qualify for our program. Once this is a go, we’ll schedule your first visit with a member of our care team. We will ensure you have adequate access to a smartphone, tablet, or computer for virtual visits. We can provide smartphones or tablets and internet access for those who qualify.

Does palliative care require hospice enrollment?

No. Palliative care is separate from hospice and may be provided while patients continue treatment for serious illness.

Can palliative care be provided at home?

Yes. Community-based palliative care services can be delivered in the home and other community settings.

Can I refer myself or a loved one?


Yes. A physician referral is not required.  Although, we might need approval from your health plan based on your benefits and coverage. If this is the case, we will let you know.
